Community Based Supports
Community Based Supports helps individuals build independence and stay connected to their community — one-on-one, wherever they need it most. Whether it's at home, at work, or out in the community, our staff provide hands-on support and training in the skills that matter for everyday life.
All 21 NJ Counties · DDD Supports Program Waiver
With Community Based Supports (CBS), a Serenity Direct Support Professional works one-on-one with each individual on the goals in their Service Plan — building practical skills and confidence wherever daily life happens, with or without a caregiver present.
What Community Based Supports Can Look Like
- Learning to cook, do laundry, or manage daily routines
- Getting support at a job site with self-care and social skills
- One-on-one tutoring or skill-building
- Help navigating the community — ordering at a restaurant, shopping, using public transit
- Support to attend classes, events, or activities of interest
CBS is tailored to each person's Service Plan and goals, with or without a caregiver present.
Community Based Supports Across New Jersey
Serenity provides Community Based Supports to adults with intellectual and developmental disabilities across New Jersey, through the NJ DDD Supports Program waiver. We are approved to support individuals with medical and behavioral acuity needs — so whatever level of support you require, we can help you build a more independent life.
