Individual Supports provides one-on-one, hands-on assistance for individuals to live more independently — in their home and out in the community. Whether it's help with daily routines or support to get out and stay connected, our staff work alongside each person to build skills and confidence, with or without a caregiver present.

Individual Supports is a one-to-one service that meets each person where they are — helping with the practical parts of daily living while building the skills and confidence that lead to greater independence.

What Individual Supports Can Look Like

Help with daily activities — dressing, personal care, meal support
Learning basic cooking, laundry, and everyday life skills
Support to attend events, take a class, or explore the community
Assistance with shopping, ordering at a restaurant, or travel training
On-the-job support with self-care and social skills

Individual Supports is coming soon to all 21 NJ counties through the DDD Community Care Program waiver.
